I have read through many of these threads but haven't been able to find what I am looking for. My husband's employer changed hands in June of 2011. He had gone to the doctor in April of 2011 and then again in late June 2011. According to him he said he told them that he didn't have his new insurance card yet, but went ahead and paid the $20.00 co-pay that we knew would be due. The $20 showed up on our bill (as a credit) and as soon as I received new Anthem insurance cards I sent in the payment for the April 2011 appointment and a copy of the Anthem card, our early 2011 insurance deducted 20% and then we were responsible for the balance up to $2500 (crappy)! SO then we get another bill that they had billed the old insurance company and that we owed more money. I had my husband contact them (while i was there) and he told them that they had billed the wrong insurance company. They asked that we send in another copy of the new insurance card and they would bill Anthem. This has been going on since then a loop that seems to be never ending. So, I am filling out bills today and check our account and see that a withdrawal for 218.84 has been pulled from our banking account from Meade and Associates, I google them and see that they are a collection agency. I have never gotten any mail from them, I haven't seen any court documents, much less a court judgement giving them the right to withdraw from our account. My husband works everyday from 7am-6pm so I am sure no one else has gotten the mail, and now I am not sure what to do. He is furious that this has happened and why the dang Dr.'s office hasn't gotten this straightened out is beyond me. I have checked to see if the claim has gone through the insurance company and it hasn't, although an appointment that he had in August went to Anthem and there has never been a problem with that one. I have been busting my butt to straighten out our credit. Especially his, since there were tons of medicals that were in his name that his ex had said were his responsibility when they were separated in 2003, I swear she went to every kind of Doctor from the time that he left their house up until 2 weeks before the divorce. Now I have something that should be so simple to fix. Sorry for rambling & if I did miss a thread like this I apologize for that as well.
Well, as far as I can figure out you and/or your husband are the victims of ID theft/fraud. You need to contact your bank, close your account and if you re-open it in the same bank make sure there are no cross over deductions. Get the actual authorization copy from the bank, put in a fraud claim with them. Opt out ( both of you) Get your credit reports To order, visit annualcreditreport.com,or call 1-877-322-8228 Check them carefully for ANY and ALL discrepancies. File a dispute, by certified mail, with each CRA correcting and/or disputing all errors, mistakes, unknown addresses, extra SS #s unknown accounts etc. File a fraud report with your police/sheriff for the illegal bank deduction File a complaint with the FTC for the fraudulent deduction.
